1.0.10.49: deadline refinements

* Deadlines are per-thread. (Children do no inherit their parents
  deadlines.)

* SIGNAL-DEADLINE estabilishes a DEFER-DEADLINE restart.

* Handle SIGNAL-DEADLINE returning due to deferred deadlines where
  necessary.

* Documentation.
